Ultrasound-Guided Central Line Placement: Practicum and Pitfalls 2018

These sessions are deliberately individualized for personal attention and coaching during hands-on procedural skills training/practice; there is one instructor for 1-2 learners per session. Session dates/times are scheduled around learner and instructor/facility availability.

Description

This course in central line placement will include didactic review of the indications and contraindications, along with discussion of the procedure and materials/equipment utilized for central line placement. The hands-on component will include review of the ultrasound equipment and practice scans of neck vasculature, preparation of the field, and mentored central line placement using procedural task trainers. Instructors will review appropriate technique and confirmation of proper placement.

Learning Objectives

Upon completion of this activity, participants will be able to:

1. Practice placing an ultrasound-guided central venous lines via the internal jugular, subclavian and femoral venous routes, with coaching and direct feedback.
2. Demonstrate the placement of an ultrasound-guided internal jugular, subclavian and femoral central venous lines on a task trainer (simulation).
3. Demonstrate correct sterile procedures while placing an ultrasound-guided central venous lines.
4. Describe the potential risks, avoidance, and treatment of complications associated with central venous line placement.

Target Audience

This activity is intended as an educational refresher and practicum for central venous line placement; allphysicians/physician assistants are eligible to apply, regardless of practice locale or domain (community and academic-based practitioners both inside and outside the Partners system are welcome to attend).

Agenda

25 Minutes

Part 1
Review of indications and contraindications. Discussion and preparation for the procedure. Reiew of the contents of the CVL kit using display board.

30 Minutes

Part 2
Review use of basic ultrasound equipment and practice scans of the neck and inguinal vasculature. Participant dons sterile gowns, glove, mask, eye protection and cap. Participant opens kit in sterile fashion and prepares the kit. Participant prepares the field by applying sterile prep solution and draping the mannequin.

45 Minutes

Part 3
Participant uses ultrasound guidance to identify and obtain needle access into the internal jugular, subclavian, and femoral vein. Participant passes the guidewire and confirms its position using 2-view ultrasound and/or manometry. Participant passes CVL catheter into the internal jugular, subclavian and femoral vein using appropriate Seldinger technique and confirms that all three lumens aspirate and infuses easily.

20 Minutes

Part 4
Participant secures CVL using sultures and catheter clamp with fastener, and applies biopatch. Participant clean the operative sute using Cloroprep, applies Mastinol skin adhesive and sterile Tegaderm dressing. Applies appropriate sterility indicator tape to the hub of the CVL.
